Full Name
The traveller’s full legal name as it appears on their official identification document.
ID Document Number
Passport or national ID card number, verified in person at check-in.
Nationality
The guest’s country of citizenship as stated in their travel document.
Date of Birth
Required for all guests over the age of 16 staying at the accommodation.
Arrival & Departure Dates
The exact check-in and check-out dates must be recorded for each stay.
Accommodation Details & NIF
The establishment name, address, and tax identification number (NIF) of the property.
How to Complete the Traveller Entry Form

Collect Guest Information
